What You'll Do (Your Impact)

As a Behavioral Health Care Manager, you will be at the forefront of integrating behavioral health into primary care, ensuring patients receive comprehensive and coordinated support for their mental and physical well-being. Your role is vital in engaging patients, conducting behavioral health assessments, providing coaching, and facilitating care coordination between providers, behavioral health consultants, and healthcare teams.



Key Responsibilities / Patient Care Coordination

Conduct behavioral health assessments using tools like PHQ-9 and GAD-7 under supervision.
Collaborate with primary care providers and behavioral health consultants to develop personalized care plans.
Perform regular patient follow-ups to track progress, remove barriers, and adjust care plans accordingly.

Patient Engagement & Education

Use motivational interviewing techniques to encourage positive behavioral changes and help patients set SMART health goals.
Educate patients about chronic conditions, behavioral health resources, and self-management strategies.

Collaboration & Communication

Serve as a trusted patient contact, ensuring seamless navigation of care services, appointments, and resources.
Maintain clear and timely communication with healthcare teams and behavioral health consultants.
Accurately document all patient interactions and care plans in the Electronic Health Record (EHR) system.

Data Tracking & Quality Improvement

Monitor and report patient outcomes, including symptom improvements and adherence to care plans.
Support quality assurance initiatives by maintaining accurate and timely documentation.
Assist in improving BHI workflows to optimize care delivery.



Success Metrics (Key Performance Indicators - KPIs)



Patient Outcomes
Demonstrated improvement in PHQ-9/GAD-7 scores.
Completion rate of documented care plans for assigned patients.



Patient Engagement & Retention
Connect with 20-22 patients per day and/or make 60 outreach calls to ensure patient engagement.
Track patient retention rates in the care management program.
Ensure successful follow-ups and active participation in care plans.



Operational & Quality Metrics
Timeliness and accuracy of documentation in the EHR.
Completion of scheduled patient interactions within expected timeframes.

Satisfaction & Team Collaboration
Positive patient satisfaction scores and testimonials.
Strong feedback from healthcare teams on collaboration and communication.



What You Need to Succeed

LVN (Compact License) or Certified Medical Assistant (MA) - Required
Experience in behavioral health, care coordination, or chronic disease management - Preferred
Strong ability to engage with patients and build trust through active listening and motivational coaching
Excellent verbal, written, and interpersonal communication skills
Proficiency in electronic health records (EHRs) and data tracking systems
Highly organized with a proactive, problem-solving mindset
Bilingual in English & Spanish (Preferred, depending on patient population)
